Sodium valproate induced hyperammonaemia in an elderly patient with affective disorder

Abstract: A 60 year old patient with bipolar affective disorder presented with clinical features suggestive of encephalopathy two weeks after commencing treatment with sodium valproate. He developed sudden onset alteration of sleep pattern, extreme agitation and hallucinations. He was disoriented in time, place, and person. Serum valproate level was within the therapeutic range. SGOT, SGPT, serum creatinine and echocardiogram were normal. Serum ammonia level which was 90 μg/dl (range: 25 - 80 μg/dl) at the time of admission had risen to 180 μg /dl on the 3rd day of hospital stay. Rapid clinical recovery from encephalopathy resulted a few days after withholding sodium valproate.

Vitamin B12 deficiency and hyperhomocysteinemia are common in tropical developing countries. In prevalence study, among Indian population by Yajnik and colleagues, 67% of men had low vitamin B12 concentration and 58% had hyperhomocysteinemia [1]. Here, we report two patients with vitamin B12 deficiency presented with symptoms of schizophrenia and required very high doses of antipsychotic medication surprisingly without any anticipated extra pyramidal side effects. The initial requirement of high dose of antipsychotic medication significantly came down after adding parenteral cobalamin supplementation.

Aim: To compare the systolic and diastolic blood pressure in both arms in elderly patients by mercury sphygmomanometer and oscillometric recorders adopting standard practice of measuring blood pressure. Materials and methods: It is a cross sectional study done in 204 elderly patients aged more than 60 years over a period of two months at teaching hospital, Jaffna, Sri Lanka. Blood pressure was measured in both arms by both devices adopting the standard practice of blood pressure measurements. Results: The mean Systolic Blood Pressure (SBP) by oscillometric recorder on both right and left arm was 4.18mmHg and 3.66 mmHg higher than mercury sphygmomanometer readings. The mean Diastolic Blood Pressure (DBP) by oscillometric recorder on both right arm and left arm was 1.67 and 1.64 mmHg lower than mercury sphygmomanometer readings. There wasn’t any significant difference in inter arm blood pressure measurement. The study group consisted of 54.3% males and 45.7% of females. The mean age of the study group was 69.46 years. Conclusion: Though there is no much difference observed in the gross values in reading the blood pressure by both oscillometric recorders and mercury sphygmomanometers, there is statistically significant discrepancy in blood pressure readings particularly in the elderly population which needs to be studied further in detail.

Abstract: Powders of strontium hexaferrite (SrFe12O19 - SrF) have been prepared by the sol-gel process. The prepared precursor was calcined in two different calcination techniques, using conventional furnace and microwave furnace. Thermal analysis studies showed exothermic and endothermic reaction peak at room temperature to 1200°C. An investigation of SrFe12O19 crystalline powder from the structural and magnetic aspect is performed using X-ray diffraction (XRD), High resolution scanning electron microscopy (HR-SEM) and vibrating sample magnetometer (VSM). The average particle diagonal size of SrFe12O19 powder was 80 - 100 nm in conventional and 40 - 70 nm in microwave calcinations respectively. XRD result showed the formation of SrFe12O19 of the sample calcined at 900oC with Fe/Sr: D-Fructose ratio = 12.

A series of polycrystalline M-type hexagonal ferrites with the formula BaFe12-2xZnx NbxO19 (x = 0.2, 0.4, 0.6, 0.8 mol%) have been synthesized by the sol-gel method. The composition and microstructure govern the magnetic properties of ferrites. The XRD analysis shows the formation of pure magneto plumbite phase without any other impurity phases. The Zn-Nb substitutions in barium hexaferrite have been confirmed through magnetic measurements. The results show that the magnetic properties are closely related to the distributions of Zn-Nb ions on the five crystallographic sites. The saturation magnetization and coercivity increase with increasing Zn-Nb concentration.

Abstract: Fungal infection usually involves the paranasal sinuses. This is a rare case of fungal mass in concha bullosa. A 19-year-old immunocompetent female patient presented with nasal obstruction. Anterior rhinoscopy revealed enlarged middle turbinate on left side. CT showed heterogenous opacity and enlargement of left middle turbinate. Intraoperatively, middle turbinate was found to be filled with cheesy material which was culture positive for Aspergillus fumigatus.
Exploring the Limits of GPUs With Parallel Graph Algorithms

Abstract: In this paper, we explore the limits of graphics processors (GPUs) for general purpose parallel computing by studying problems that require highly irregular data access patterns: parallel graph algorithms for list ranking and connected components. Such graph problems represent a worst case scenario for coalescing parallel memory accesses on GPUs which is critical for good GPU performance. Our experimental study indicates that PRAM algorithms are a good starting point for developing efficient parallel GPU methods but require non-trivial modifications to ensure good GPU performance. We present a set of guidelines that help algorithm designers adapt PRAM graph algorithms for parallel GPU computation. We point out that the study of parallel graph algorithms for GPUs is of wider interest for discrete and combinatorial problems in general because many of these problems require similar irregular data access patterns.
