Avalia??o da mortalidade de neonatos e crian?as relacionada ao uso do cateter venoso central: revis?o sistemática

Abstract: objective: to analyze mortality related to infections in the use of central venous catheters in newborns and hospitalized children. methods: this was a systematic review, using studies that were identified in the databases of medline, embase, lilacs, cinahl, scielo and cochrane, in bibliographical references of articles, and other reviews. two reviewers independently identified relevant studies, analyzed the methodological quality, and subsequently, extracted data. results: we encountered 1,000 articles, of which 16 were related to catheter infection and only two mentioned mortality. findings of these two studies verified that sepsis was the principal complication related to the use of central venous catheters, and the most prevalent microorganisms in these infections were candida sp. and enterococcus sp., with significant impact on neonatal and child mortality. conclusion: it was observed in this review that sepsis was most prevalent complication related to central catheters.

Abstract: O Centro de Medicina do Idoso (CMI) do Hospital Universitário de Brasília (HUB), referência no atendimento a saúde dos idosos, enfoca a terceira idade dentro de um contexto multidisciplinar no qual a Odontologia encontra-se inserida. O Projeto de Extens o de A o Contínua “Aten o odontológica aos pacientes do CMI-HUB” vem sendo desenvolvido desde o ano de 2004, visando realizar orienta es em saúde bucal voltadas à popula o senil, particularmente acometida por quadros demenciais, dentre eles a Doen a de Alzheimer. O presente trabalho tem por finalidade apresentar as principais a es realizadas por odontólogos extensionistas, orienta es aos cuidadores e familiares dos idosos a respeito da promo o de saúde bucal. Ressalta-se o importante papel do cirurgi o-dentista como parte integrante da equipe multidisciplinar junto a este público.

Abstract: objective: to conduct a systematic review of the literature on the prevention of catheter-related bloodstream infections in patients on hemodialysis. methods: a literature search was performed in medline, embase, scielo, lilacs, and cochrane databases for articles published from 1990 to 2008. results: two hundred ninety- three articles were retrieved. however, only 12 studies were included in this review. conclusions: findings suggest that health care providers use several infection control measures to prevent bloodstream infections in this patient population.

Abstract: objective: to evaluate the impact of the use of topical mupirocin on the insertion of central venous catheter for hemodialysis. methods: this was a systematic review with meta-analysis. results: after a careful and extensive search, we included three clinical trials that compared the use of mupirocin versus other intervention in central venous catheter for hemodialysis. conclusion: the study found that the use of topical mupirocin is effective in reducing episodes of infection among hemodialysis patients, increasing duration time for catheter, and significantly reducing s aureus infections, which are the most prevalent in this population.

Abstract: infection with group b streptococcus (gbs) is considered an important public health problem. it is associated with: neonatal sepsis, meningitis, pneumonia, neonatal death, septic abortion, chorioamnionitis, endometritis and other perinatal infections. the aim of this study was to determine the best screening strategy for gbs in pregnant women. for this a systematic review and meta-analysis were carried out in the nursing department of the federal university of s?o paulo, cochrane center, brazil. sources used were, embase, lilacs, medline, list of references, personal communication and the cochrane library. the criterion for the selection of the studies was; studies which analyze some type of screening for gbs in pregnant women. independent of the comparator, all analyses were in favor of a universal screening program for reducing the incidence of neonatal sepsis. the evidence obtained in this study suggests that the strategy of universal screening of pregnant women associated with the use of prophylactic antibiotics is safe and effective.

Abstract: the results of the studies about foci of schistosomiasis in the localities of pedro de toledo and itariri, are presented with special reference to the natural infection rate, reached for the biomphalaria tenagophila species. this snails which transmits schistosomiasis in "vale do ribeira", was also found in the following counties in that region of the state of s?o paulo: eldorado, iguape, itariri, jacupiranga, juquiá, miracatu, pariquera?u, pedro de toledo, registro and sete barras. the natural infection rates of 68.1%; 75.0%; and 81.3% obtained in some samples of b. tenagophila, the highest till now verified of this species, prove once more the great susceptibility to the infection by schistosoma mansoni.
A combination therapy of ethanol injection and radiofrequency ablation under general anesthesia for the treatment of hepatocellular carcinoma

Abstract: AIM: To summarize the effects of laparoscopic ethanol injection and radiofrequency ablation (L-EI-RFA), thoracoscopic (T-EI-RFA) and open-surgery assisted EI-RFA (O-EI-RFA) under general anesthesia for the treatment of hepatocellular carcinoma (HCC).METHODS: Time-lag performance of RFA after ethanol injection (Time-lag PEI-RFA) was performed in all cases. The volume of coagulated necrosis and the applied energy for total and per unit volume coagulated necrosis were examined in the groups treated under general (group G) or local anesthesia (group L).RESULTS: The results showed that the total applied energy and the applied energy per unit volume of whole and marginal, coagulated necrosis were significantly larger in group G than those in the group L, resulting in a larger volume of coagulated necrosis in the group G. The rate of local tumor recurrence within one year was extremely low in group G.CONCLUSION: These results suggest that EI-RFA, under general anesthesia, may be effective for the treatment of HCC because a larger quantity of ethanol and energy could be applied during treatment under pain-free condition for the patients.
Cross-transmission of vancomycin-resistant Enterococcus in patients undergoing dialysis and kidney transplant

Abstract: the objective of this study was to investigate the occurrence of vancomycin-resistant enterococcus (vre) cross-transmission between two patient groups (long-term dialysis and kidney transplant patients). molecular typing, by automated ribotyping with the riboprinter microbial characterization system (qualicon, usa), was used to analyze vre isolates from 31 fecal samples of 320 dialysis patients and 38 fecal samples of 280 kidney transplant patients. clonal spread of e. faecalis and e. casseliflavus was observed intragroup, but not between the two groups of patients. in turn, transmission of e. gallinarum and e. faecium between the groups was suggested by the finding of vancomycin-resistant isolates belonging to the same ribogroup in both dialysis and transplant patients. the fact that these patients were colonized by vre from the same ribogroup in the same health care facility provides evidence for cross-transmission and supports the adoption of stringent infection control measures to prevent dissemination of these bacteria.
